This public record is already selected. Review it on its own, or add one to three more communities for a side-by-side comparison.
Review this community free →The comparison and private planning tools stay available while Haven’s placement-request channel is closed.
Free to use. Opening the comparison sends no family details, contacts no community, and creates no referral. Any exact-community introduction requires separate authorization; disclose an earlier direct inquiry before authorizing one.
Research with someone you trust
Send this same source-labeled profile to family or a care professional. No signup is needed, and the link includes no shortlist, private notes, contact details, or Haven referral. Haven does not receive a recipient or message.
The link itself identifies no one. A fixed, non-personal source label measures aggregate shared-profile visits. If someone who opens it later requests Haven’s help, that label may accompany the request. Haven never receives the share recipient or message.
Build a private 2–4 community shortlist
Save this profile, then compare the same written answers across your finalists. This list stays in this browser and is not a Haven recommendation.
Information-only next step
Use these public details to ask for current written terms. Haven does not collect or send your information here, and this is not a Haven referral, availability confirmation, care-fit decision, or placement promise.
Ask for the same written basics without putting a diagnosis, medication list, or other private details into an ordinary contact form.
After your call
Opening this does not add the community to your shortlist or auto-fill a call, date, stage, quote, or written answer. It restores only anything this browser already saved.
Side-by-side research
Each option opens the same two-community view of public, source-labeled records. Choose the record you want to put beside this one.
This step uses public community records only. It does not collect, submit, or send your information.

Haven separates evidence published on this page from facts that can change daily. Save or print this checklist so every community answers the same questions.
No signup or family information is requested. Haven does not receive the brief contents or the browser, printer, PDF, or save destination you choose.
This page includes a local market benchmark, not a rate from this community.
Source & scope: A market benchmark helps orient a search, but it cannot tell you this community's all-in cost.
Not verified: no community-specific Utah New Choices funding-path policy is published for this community.
Source & scope: A Utah license, price, or care listing does not establish waiver participation or coverage. Do not treat missing evidence as a negative answer.
This profile lists Assisted Living.
Source & scope: The state record supports the licensed provider identity or license type, not a promise that a particular care program is available today. A listed care type does not confirm staffing, accommodations, or suitability for one person's needs.
This profile does not publish a live availability or waitlist feed.
Source & scope: Do not infer an opening from licensed capacity, historical census, or a listing's presence on Haven. Availability can change daily.

Source & scope: Utah DHHS DLBC public provider portal snapshot retrieved Aug 23, 2026. It is a dated public record, not a safety score or a guarantee that all current facts are complete.
No individually linked third-party listing snapshot is published on this profile.
Source & scope: Each card is a separately linked third-party listing snapshot, not a Haven aggregate or verified-family review. Review counts and ratings do not verify current staffing, safety, availability, or fit for your loved one.
Not verified: this facility record does not include source-dated residency-document availability.
Source & scope: A listing, review, rate, or regulator record does not replace the terms you will be asked to sign. Review the actual documents before paying a deposit.
Haven does not treat a score, capacity, rating, license, or review as a guarantee of current availability, quality, safety, or suitability. Compare written answers from at least two communities before signing.
Copy this request and send it yourself when you are ready. Haven does not contact a community, share information, or promise to retrieve documents through this tool.
Before a tour or care-fit call
A community's own assessment process determines whether it can safely support one person's needs. A listing and a tour cannot answer that. Bring a short, current picture of daily support needs so every community evaluates the same facts.
Keep medical records private: do not put medication lists, diagnoses, or documents in Haven's general request form. Ask the community which secure process it uses for an assessment. This is preparation, not medical or legal advice.
Community-published rates and editor-approved community submissions are labeled separately from market benchmarks. A benchmark is not this community's price or an all-in quote.
| Level of care | Monthly cost | Source |
|---|---|---|
| Assisted Living | $3,820–$5,980/mo (benchmark median $4,780) | Market benchmark — not this community's rate |
Facility-specific starting rates preserve every captured source-stated care and fee qualification in their row. A starting rate is not a current all-in quote. Market benchmarks are not rates from this community. How we estimate costs
Before comparing this community with another, ask each one to put the same items in writing for the residence and care your family needs.

Most families start this search after a fall, a hospital stay, or a caregiver reaching a breaking point — and end up choosing from whatever has a bed open that week. Families who plan even a few weeks ahead get to choose on quality, fit, and price instead of availability alone.
Source labels identify who supplied a fact. They are not a blanket endorsement, live-availability signal, or paid placement. Participation never changes an official record, source label, or evidence review.
