How we present senior-care records

CMS publishes separate overall, health-inspection, staffing, and quality- measure stars for eligible Medicare- and Medicaid-certified nursing homes. Haven shows those federal fields with their source and scope rather than weighting or aggregating them into an in-house score.

A source record cannot establish a current opening, care fit, funding approval, or family-specific recommendation. State assisted-living records are not converted into federal CMS ratings, and public rosters use neutral name order.

• CMS Care Compare — federal ratings, staffing, inspection deficiencies, fines, and ownership for skilled nursing facilities. Updated periodically from the official dataset.
• State licensing agencies— assisted living and memory care are licensed at the state level. We ingest the official registries where a source and coverage scope are explicitly disclosed. The currently verified assisted-living public-record layer is Utah DLBC Type I/II coverage for Salt Lake County. We do not infer that a listing in another market is current or complete without equivalent source verification. “Residential care home” is an editorial label, not a universal regulator category.
• Verified pricing— confirmed with communities directly, shown with a clear source and confidence label. Where we don't yet have a community-published or directly verified price, we show a market benchmark and label it as a benchmark, not a facility quote.
• First-party verification — a fact is marked only when Haven has recorded its source and verification date; otherwise it is shown as unknown or as a clearly labeled market estimate.

In Salt Lake County, Utah, Haven matches each in-scope assisted-living community to the Utah DHHS Division of Licensing and Background Checks public provider portal using the regulator's durable license ID. We show the license type, capacity, secured-bed field where published, current public-record status, and eligible public inspection findings with a direct source link and retrieval date.

This is not a safety score. Public DLBC history is limited, records or findings under appeal are excluded, and no displayed finding does not establish safety or quality. Utah records are never converted into CMS stars, F-tags, staffing measures, or a Haven quality score.

CMS nursing-home facts remain record-by-record: stars, staffing, inspection history, fines, abuse icons, and Special Focus status each retain their federal source context. We do not publish a Haven leadership board or turn these facts into a universal senior-care recommendation.

We publish only reviews we can verify came from a real resident, family member, or recent visitor. We do not buy reviews, post anonymous unverifiable ratings, or let communities edit or remove honest feedback. This follows FTC guidance on review authenticity.

Haven's public research tools are free for families. No family-to-community handoff or placement fee is active through this deployment. Source-labeled records retain their documented source and scope.

Where Haven states that a market has complete official roster coverage, we include every in-scope licensed community whether or not it works with us. Elsewhere, we disclose the source and coverage instead of implying completeness. Paying partners may get enhanced profiles, but they cannot alter a source label, remove unfavorable source facts, or buy a favorable recommendation.

Senior care details change. Each profile's Data status & corrections panel identifies the source and date associated with what we show, and lets a community, family member, or visitor submit a fact for evidence review. A submission never edits a profile automatically, removes an official record, or changes how a source-labeled public record is shown. We check supporting evidence before a catalog update; official regulator records remain attributable to their regulator. Always confirm time-sensitive specifics directly with the community before making a decision.
