Review Haven's Utah placement pilot before protected intake opens.

Use this public, no-signup page to match an eligible Utah Assisted Living Facility Type I/II record, review the pilot terms, and prepare source-dated profile facts. Protected intake is closed, so Haven accepts no private provider information here.

Today this deployment is reference-only: public-record matching and display-only agreement packets remain available for preparation, but Haven does not accept an online provider signup, signature, referral, payment, or placement-fee activation.

Reference-only profile preparation

Any community can prepare current facility-level price, care, and availability facts for a later protected evidence review. Preparation is free and does not change source-labeled public records or neutral listing order.

Clear separation from partnerships

Profile verification is editorial evidence review. It does not create a placement agreement, alter inclusion, or buy a favorable recommendation.

Prepare evidence before later review

Use the public checklist to prepare non-sensitive, source-dated facts. Haven is not accepting a source, contact, document, or fact through this deployment today.

Reference-only Utah scope

The public agreement-review packet is limited to eligible Utah communities that are canonical Assisted Living Facility Type I/II records. Outside Utah, use the reference-only profile-preparation path below; it does not create a placement partnership, referral, or placement-fee claim.

Prepare for a later protected profile review

  1. 1
    Find the right public record
Use the Utah picker or public directory to confirm the community that the future evidence review would concern. A record match does not submit anything.
  2. 2
    Prepare source-dated facts
Use the checklist below to prepare a current price, care, or availability fact with its source and effective date. Do not put any fact, contact, document, or resident information into this public page.
  3. 3
    Wait for protected intake
Haven is not accepting profile verification through this deployment today. A later protected review would still check scope, source, and date before any public update.
  4. 4
    Families keep the context
Any future public information keeps its source and date, so families can distinguish an official record, community statement, and market estimate.
Utah placement-partnership pilot

Placement-partnership terms at a glance

This is a transparent, performance-based referral arrangement for eligible Utah communities—not a paid listing product. The exact versioned agreement controls, and Haven must complete signer-authority and compliance review before any relationship becomes active.

Today's public path is reference-only: it does not accept an online provider signup, signature, referral, payment, or placement-fee activation. You can still review the terms, select an eligible Utah record, and prepare public-profile facts for a later protected review.

Pay only for a completed, documented outcome
There is no upfront listing fee or pay-per-lead charge. A one-time fee is considered only after an approved agreement, a family-authorized Haven referral, and the family's physical move-in.
Private-pay base rent only
The current pilot fee is one month's private-pay base rent. It excludes care packages, deposits, community and move-in fees, and placements funded in whole or part at admission by Medicare, Medicaid, or another public health-care program.
Clear attribution, billing, and refund protections
The agreement sets the attribution window and prior-inquiry process. Haven coordinates an itemized manual invoice due 30 days after move-in, with first-30-day refund or credit protections documented in the agreement.
No paid recommendation or referral guarantee
A partnership cannot buy a favorable recommendation, change a source label, or alter an official record. It is non-exclusive and does not promise a minimum number of referrals, tours, or move-ins.

Reference-only today. This picker helps identify a public Type I/II record and its exact agreement packet; it is not commercial approval. Searching, opening a record, or reviewing or printing a packet does not send Haven an online provider signup, signature, work contact, application, referral, payment, or placement-fee activation.

This reference-only agreement-review path is limited to eligible Utah records. You can still prepare a free public-profile evidence packet. Preparation is display-only: it does not submit a fact, contact, source, or document for review, and it remains separate from placement participation.

Utah placement-partnership questions, answered plainly

Review the boundaries before involving a signer. This information does not create an application, referral, agreement, or fee claim; the versioned agreement controls if this summary differs.

Which communities does Haven's Utah placement-partnership pilot cover?

The current public agreement is limited to an eligible Utah senior living community matched to a canonical Haven directory record. An authorized signer must bind the legal owner or operator, and Haven must complete its countersignature and compliance review before any relationship is active.

Is this a paid listing or pay-per-lead program?

No. The Utah pilot has no upfront listing fee or pay-per-lead charge. Under the exact agreement, a one-time placement fee is considered only after an executed agreement, a family-authorized Haven referral, and a documented private-pay move-in.

What is included in Haven's placement-fee calculation?

The current agreement sets the fee at one month's private-pay Base Monthly Rent for the first full month. It excludes care packages, deposits, community and move-in fees, and other ancillary charges. No fee is owed for a move-in funded in whole or part at admission by Medicare, Medicaid, or another public health-care program.

Can a partnership change a community's public record or guarantee referrals?

No. A partnership cannot buy a favorable recommendation, change a source label, or alter an official record. The relationship is non-exclusive and does not promise a minimum number of referrals, tours, or move-ins.

Does a Haven-referred family pay more?

No. Families never pay Haven. The agreement requires participating communities to offer referred families the same rates, discounts, and concessions available to comparable direct move-ins; a community cannot surcharge a family to recover Haven's fee.

What can a Utah community do today?

Today, an eligible Utah community can match its public directory record and review, print, or share its exact public agreement reference packet internally. This deployment is reference-only: it does not accept an online provider signup, signature, referral, payment, or placement-fee activation. Communities outside Utah can prepare source-dated profile facts for a later free editorial review; that work remains separate from a placement partnership and does not change a public source record or neutral listing order.

Provider intake is not active in this deployment. This is a display-only preparation path: Haven is not requesting or accepting a source, contact, document, or fact here. The checklist below can help an operator prepare non-sensitive, source-dated facts for a later protected review; it remains separate from any placement partnership.

Prepare a free profile-evidence packet

Get source-dated facts ready without sending anything

Secure provider intake is not active in this deployment, so Haven is not requesting a work contact, source link, document, or fact here. You can still prepare a short internal packet now so a later protected evidence review can start with the right public-record context and current source dates.

What to prepare for a later evidence review

Keep each fact with the source that supports it and that source's effective or as-of date. If facts come from different sources or dates, keep them separate rather than combining them into one claim.

Current price and mandatory fees
Base rent, mandatory care charges, and common required fees.
A reviewed price source is current for public use for 90 days from its stated effective date.
Care capabilities and policies
What the community can consider after its own assessment.
A reviewed community policy or capability source is current for public use for 180 days from its stated effective date.
Current availability
An as-of-date only; it is never treated as a live inventory feed.
A dated availability snapshot is never live inventory and is current for public use for only 7 days from its as-of date.
Residency documents and exit terms
Which current documents or policies exist; never the contract text, legal advice, or a ranking signal.
A reviewed residency-document or policy label is current for public use for 180 days from its stated effective date.

Keep the packet safe and decision-useful

  • For pricing, identify the exact care setting, base rent, mandatory charges, and whether an exact base rent, range, or no exact figure may be considered for public display. Base rent is not an all-in quote.
  • For care, residency terms, and Utah funding policy, prepare only facility-level policies. A community's policy never decides an individual's admission, care fit, eligibility, or coverage.
  • For availability, write an as-of date and treat it as a dated snapshot, never a promise of an opening, acceptance, tour, or move-in.
  • Do not include resident or family names, care plans, diagnoses, medication lists, financial details, signed residency agreements, portal credentials, or any document containing private information.

Share a privacy-safe team handoff

Copy the generic readiness page for an owner, operations, marketing, or compliance colleague. The link contains no selected community name, record ID, source, document, contact, or private information. It is a preparation aid only, not a request, approval, or partner application.

Printing this preparation aid does not submit evidence, open a review, change a public profile, improve rank, activate a partnership, send a referral, or create a placement-fee claim. Free profile verification remains separate from any agreement or commercial relationship.
